Privacy Policy
Last updated: January 2026
Scope
This policy explains how Epic Medical Centers handles information collected through this website, our online intake form, and our text and phone communications. It does not replace the Notice of Privacy Practices you receive as a patient at one of our clinics, which governs protected health information under HIPAA.
Information we collect
- Contact details you submit: name, phone number, email address.
- Injury details you choose to share: injury type, date of injury, insurance or claim number, preferred clinic, and your description of what happened.
- Communication preferences, including whether you consented to receive text messages.
- Basic technical data such as pages visited and general location derived from your IP address, used to keep the site secure and functional.
How we use it
- To contact you about scheduling, confirming, or following up on an appointment.
- To route your request to the clinic closest to you.
- To verify insurance or claim coverage before your visit.
- To respond to questions you send us.
- To meet legal, regulatory, and recordkeeping obligations.
How we share it
We share information with the Epic Medical Centers clinic handling your care, and with service providers who operate our scheduling, hosting, and messaging systems under contract. We may disclose information when required by law, court order, or to protect the safety of a person.
We do not sell your personal information, and we do not share it with advertisers for their own marketing purposes.
Text messages
Mobile opt-in data and consent are never shared with third parties for marketing. See our SMS Terms for the full messaging policy, including how to opt out.
Retention and security
We keep submitted information as long as needed to provide care, meet legal requirements, and resolve disputes. We use administrative, technical, and physical safeguards to protect it. No system is perfectly secure, so please avoid sending sensitive details you would not want transmitted electronically.
Your choices
- Ask us to correct or delete information you submitted through the website.
- Withdraw text-message consent at any time by replying STOP.
- Request a copy of your clinical records through the treating clinic.
Children
This website is not directed to children under 13, and we do not knowingly collect their information through it. Minor patients are treated only with the consent of a parent or legal guardian.
